"Medical/Dental, Affordable Health Insurance"

Does anyone have solid factual information about the organizational structure(s) behind the various signs promoting some form of medical, dental, and/or health insurance? I know that NASE, the National Association for the Self-Employed, promotes some of this type of insurance. Also, MEGA Insurance has been mentioned. I think it is likely that there are several unrelated national organizations as well as people operating on their own, perhaps just local insurance agents who are using illegal signs but otherwise act responsibly.

Is this operated as a MLM? How do people who put out signs get compensated? Are there promotional kits that someone puts together and sells? What kind of insurance products do they have? Does anyone have experience with responding to a promotion and seeing what is offerred?

Since this type of spam seems to be the next largest problem )after HerbaLife) we probably need to understand what is behind the signs so we can best attack the problem.

What facts can people contribute?

(Let's try to keep away from simple claims of "scam.")
